This thing is far beyond ultra rare. Several people report not getting it after multiple thousand kills. Note: this may be because players are killing mobs listed on this page that don't have the possibility to drop it. Thousands of shambling cubes have never dropped a shawl, but I have had multiple shawls drop for myself and others off of Fe`Dhar, high level named Wurms (e.g. Jaelk, Lararith) and Oct Velic. Presumably the other high level Velics can drop Fe`Dhar loot, but if you're running solo you won't be able to kill them.
